A.  BACKGROUND

  1. Health Status: The health status of the approximately 32,200 million Afghans is among the worst in the world. The infant mortality rate is 50 per 1,000 live births and the maternal mortality ratio is 396/100,000 live births. The nutritional status of children and women is very poor. The poor health status of women and children is exacerbated by a very high fertility rate and low contraceptive prevalence. Most of the burden of diseases results from infectious causes, particularly among children where diarrhea, acute respiratory infections, and vaccine preventable illnesses likely account for 60% of deaths.
  2. Health Care Services: Until 2003 the performance of the public health system was poor (e.g. only 8% of women received prenatal care) with the exception of mass vaccination campaigns which achieved high coverage. Since 2004, there has been a large increase in service delivery, about 90% of it provided by NGOs. The Ministry of Public Health (MoPH) has taken the responsibility to organize the allocation of funds within the sector and to set guidelines and standards for service provisions by NGOs, which are contracted. The Basic Package of Health Services (BPHS) and Essential Package of Health Services (EPHS) have been developed which clearly delineate the service that should be provided by each type of primary health care and hospital facilities respectively.
  3. Health System :

Despite impressive progress in health system development and services delivery, Afghanistan still faces tremendous challenges in provision of equable quality health care. In addition to security and dependency on external support, challenges such as improved access to health care, up grading the skills of service providers, weak demand for services utilization, and gaps in stewardship role of the ministry such as properly monitoring the system at all levels, are example of these challenges. In order to strengthen the system and address the bottlenecks, the Global Alliance on Vaccination and Immunization (GAVI) opened an opportunity to a group of countries to fill the gaps and strengthen their health systems.

  1. MoPH response:

Afghanistan submitted its proposal in accordance with the donor guidelines, proposing some of the critical areas that affect the system. These areas mainly include, improved access to quality health care, increased demand for health services utilization and strengthen stewardship role of the MOPH at different levels.    

OBJECTIVES

 (i)  provide the Ministry of Public Health in general and (Monitoring) Directorate and Health System Strengthening (HSS) in special with the ability to effectively monitor and evaluate the project’s benchmarks (ii) to assist the project implementers to find out the gaps in performance (iii) to help the project implementers to take remedial actions; and (v) build the capacity of implementer staff to carry out the project monitoring and measuring the progress of the project..

  Duties and Responsibilities:         

The community based monitoring consultant is reporting to the director of the Monitoring directorate.

Work during the official times with the MoPH.

He/She is responsible for the following:

    • To develop required Monitoring and Evaluation tools e.g. guidelines SoPs for community based monitoring system in close collaboration with monitoring directorate.
    • Provide technical assistance and conduct needed M&E trainings to community supervisors and community monitors involved in implementing Monitoring activities in this area.
    •   Performance management and performance appraisal of community supervisors and community monitors according to regular reporting information, monitoring missions and other assessment mechanisms.
    • Coordinate and facilitate payment process for community supervisors and community monitors with finance department.
    • Ensure that community based monitoring is done in compliance with the M&E rules and regulation in targeted provinces.
    • Compile the monitoring reports of all community base supervisors and community monitors involved in the implementation of CBMs.
    • Allocate 40% of time for monitoring of targeted (hard to reach and high risk polio) provinces.
    • Carry out any other job related business coming from the disk of line supervisor.  

LOCATION AND DURATION OF SERVICES

  1. The community based monitoring consultant will be based in Kabul, with frequent  travels to  targeted provinces to conduct  supervision, monitoring and evaluation and to provide training and support the provincial staff (PEMT, REMT, community supervisors, community monitors). It is expected that the carrying the job will have duration of one year which may extended for 10 months.

 QUALIFICATIONS AND EXPERIENCE

  1.       The successful applicants will have:
  • To be MD doctor,  preferable with master degree in public health
  • To have 3 years’ working experience in the field of monitoring and evaluation and 4 years’ experience in public health with MOPH, NGOs and UN agencies
  • Be able to travel to provinces
  • Fluency in English language both speaking and writing.
  • A demonstrated ability to get along with people and deal with disagreement.

The MOPH will provide the employee relevant information related to the activities to be carried out, and will ensure that staff, at he central and provincial levels, are available for periodic meeting/workshop.

The MOPH will make available to the employee necessary facilities in order to do her or his job perfectly.
