   Job Purpose / Objective:

The Chief of the Nephrology and Dialysis Department holds strategic leadership and is responsible for delivering high-quality clinical care in the fields of nephrology, renal replacement therapy, and dialysis services. This position encompasses the management of clinical, educational, research, and administrative activities, as well as the mentoring of trainees, fellows, and nursing staff within the department, to ensure compliance with the highest standards of patient care.

The Chief of the Nephrology and Dialysis Department will work in close collaboration with chiefs of other departments, hospital units, senior management, and healthcare personnel to ensure that departmental activities are aligned with the hospital’s mission, vision, and overall objectives.    .................................................................................................................................

Duties and Responsibilities:

  Core Duties:

     Clinical Leadership:

  1. Lead and manage all nephrology and dialysis services with a focus on patient-centered care across inpatient, ward, and critical care settings.
  2. Diagnose and manage acute and chronic kidney diseases, electrolyte disturbances, and hypertension.
  3. Oversee the quality of dialysis services, vascular access management, and other renal replacement therapies for patients with kidney diseases, including renal failure, dialysis, and chronic kidney disease (CKD).
  4. Ensure proper use, maintenance, and safety of dialysis equipment.
  5. Participate in and collaborate with clinical rounds in critical care units and nephrology consultations.
  6. Manage complex nephrology and dialysis cases and provide expert consultation to other departments.
  7. Effectively manage dialysis services, including hemodialysis and peritoneal dialysis.
  8. Supervise and collaborate with fellows, trainee doctors, nurses, and other healthcare staff to ensure the provision of high-quality care.
  9. Ensure efficient patient flow, timely interventions, and adherence to protocols and hospital quality standards.
  10. Develop, implement, and maintain departmental protocols, guidelines, and evidence-based best practices.
  11. Keep up-to-date with advancements in nephrology and dialysis care and integrate innovations, technologies, and diagnostic and therapeutic procedures.
  12. Review departmental performance indicators, such as patient outcomes, and implement corrective actions to improve service delivery.
  13. Participate in on-call duties as Chief and, if needed, second-call according to the departmental and hospital duty roster.
  14. Ensure timely completion of consultations and professional advice.

   Education and Capacity Building:

  1. Lead and supervise the training of fellows, trainees, and medical students in the Nephrology and Dialysis Department to ensure high-quality clinical and academic education.
  2. Promote a culture of continuous learning and professional development.
  3. Organize Continuing Medical Education (CME) programs to keep the medical team up-to-date.
  4. Facilitate clinical teaching and mentorship to strengthen a culture of education and professional growth.
  5. Design, implement, and oversee educational programs, workshops, lectures, scientific and professional discussions, and conferences to enhance the capacity of department staff.
  6. Participate in patient rounds, case discussions, and seminars.
  7. Ensure that departmental educational programs align with the hospital’s academic and training objectives.

   Research and Scientific Excellence:

  1. Design, identify, support, and implement clinical and experimental research topics based on clinical needs and research priorities of the hospital and the Ministry of Public Health within the department.
  2. Secure necessary resources and funding for research projects and ensure adherence to ethical standards at all stages of research.
  3. Encourage department members to publish articles and research findings in reputable national, regional, and international journals to maintain up-to-date knowledge in nephrology and dialysis.
  4. Lead or participate in clinical research and quality improvement projects.
  5. Organize weekly scientific meetings, conferences, and research seminars to promote and enhance the scientific and research skills of departmental staff.
  6. Continuously monitor, analyze, and evaluate the quality of research activities, outcomes, and achievements, and provide recommendations for process improvements within the department.

   Leadership and Administrative Responsibilities:

  1. Responsible for managing all professional and administrative operations, including recruitment, onboarding, performance evaluation, and professional development of all departmental medical personnel.
  2. Provide leadership, mentorship, and professional development opportunities for all departmental staff.
  3. Develop and implement monthly, quarterly, annual, and strategic departmental work plans in alignment with the hospital’s operational plan and the Ministry of Public Health strategic plan to improve departmental performance, clinical outcomes, and patient satisfaction.
  4. Manage the departmental budget and resources to ensure proper allocation and cost-effectiveness.
  5. Supervise inventory, procurement, and utilization of nephrology and dialysis-related supplies and equipment.
  6. Collaborate with hospital leadership to ensure departmental compliance with regulations, accreditation standards, and quality indicators.
  7. Evaluate departmental performance and implement corrective actions as necessary to achieve quality, safety, and patient satisfaction standards.
  8. Arrange and maintain 24-hour duty rosters for departmental professional staff, including specialist trainers, trainees, technicians, and fellows, including leave and public holidays according to scheduled shifts.
  9. Actively participate in morning reports to ensure proper service delivery over the past 24 hours in the department.
  10. Ensure that all departmental personnel, including on-call staff, are present and performing their assigned duties.
  11. Collaborate with the hospital management team in implementing decisions, staff allocations, and workflow management.
  12. Ensure the implementation of clinical guidelines, standards, professional procedures, and diagnostic and treatment protocols to enhance nephrology and dialysis service quality.
  13. Be prepared for and participate in hospital emergency events to provide timely nephrology and dialysis care to patients in need.
  14. Participate in hospital-wide clinical rounds (Round Visit Hospital), DTC committee meetings, mortality review committees, quality improvement committees, HMIS committees, and other sessions convened by the hospital administration.
  15. Supervise and manage disciplinary matters within the department.
  16. Provide monthly, quarterly, annual, and ad-hoc reports on departmental activities and achievements to the Hospital Chief Medical Officer.
  17. Perform other duties assigned by the hospital administration in accordance with laws, regulations, and the objectives of the hospital and the Ministry of Public Health.

   Quality Assurance and Patient Safety:

  1. Promote and foster a culture of patient safety and continuous quality improvement in patient care and workflow within the department.
  2. Ensure that the Nephrology and Dialysis Department fully complies with all health regulations, safety standards, and accreditation requirements.
  3. Maintain key quality indicators such as dialysis adequacy, infection rates, and patient outcomes.
  4. Ensure adherence to clinical quality standards and patient safety protocols within the department.
  5. Lead efforts and initiatives for Continuous Quality Improvement (CQI) and monitor departmental performance against established benchmarks.
  6. Collaborate with hospital administration and other quality improvement committees to address issues related to patient safety, clinical care, and hospital-acquired infections (HAI).
  7. Conduct regular audits of surgical outcomes and procedures and implement corrective actions as necessary.
  8. Lead morbidity and complication review sessions to enhance departmental performance.
  9. Maintain accreditation standards, comply with national health regulations, and ensure readiness for external evaluations.

   Collaboration and Communication:

  1. Maintain close collaboration with other department chiefs, unit heads, senior leadership, and support services to ensure uninterrupted, comprehensive, and integrated patient care across the hospital.
  2. Participate in all hospital-wide committees and provide expert input for strategic planning, operational goals, and hospital policy development.
  3. Foster a collaborative, supportive, and team-oriented work environment among faculty, staff, and trainees within the department.
  4. Ensure open and effective communication at all departmental levels, promoting a culture of transparency, trust, and mutual respect.
  5. Establish and maintain strong working relationships with other clinical departments, including Trauma, Emergency Medicine, Radiology, Critical Care, and Anesthesia, to provide coordinated and integrated care for nephrology and dialysis patients.

Patient and Family Relations:

  1. Promote and support patient-centered care, including providing clear and comprehensive information to patients and their families, and actively involving them in treatment decisions.
  2. Address patient and family complaints and concerns regarding nephrology and dialysis services promptly and effectively.
  3. Enhance the hospital’s reputation as a provider of high-quality and compassionate care within the community.

Employment Requirements / Qualifications:

1. Citizenship: Must be a citizen of Afghanistan.

2. Age: Must be between 35 and 60 years of age.

3. Educational Qualifications:

  • Medical Doctor (MD) or equivalent, with completed specialty training in Nephrology (MD/DNB/FCPS) from a recognized national or international university or specialist training institution, accredited by the Ministry of Higher Education.
  • Advanced training certification or fellowship and a recognized specialty board certification in Nephrology or equivalent is preferred.

4. Work Experience:

  • Minimum 5 to 7 years of clinical experience in Nephrology, including dialysis management.
  • At least 3 to 5 years of experience in a leadership position in a teaching hospital or specialized clinical services is preferred.
  • Experience in training, mentoring, and supervising medical students, residents, or fellows.
  • Experience in managing multidisciplinary healthcare teams.

5. Professional License:

  • Must hold a valid medical practice license issued by the Medical Council.

6. Skills and Competencies:

  • Proficiency in one of the official national languages (Pashto or Dari).
  • Computer literacy in programs relevant to the position.
  • Advanced knowledge in clinical care and treatment in Nephrology and Dialysis.
  • Ability to train, guide, and supervise trainees in the Postgraduate Medical Education (PGME) program.
  • Thorough understanding of hospital quality standards, patient safety protocols, quality improvement, and clinical audit procedures.
  • Excellent teaching, interpersonal communication, and mentorship skills.
  • Extensive knowledge of Nephrology and Dialysis, medical ethics, clinical research, and academic medicine.
  • Ability to manage emergency situations effectively and availability for on-call duties as required.
  • Proficiency in English (speaking, writing, and reading).
  • Strategic planning skills and effective implementation of departmental objectives.
  • Commitment to innovation, academic excellence, and patient-centered care.
  • Integrity and professionalism in all interactions with patients, families, and hospital staff.
  • Ability to accept mistakes, take responsibility, and work effectively in multidisciplinary teams.
  • Ability to maintain composure and calm under critical or high-pressure situations.
